Complete the Right Forms

Paperwork is always required when hiring outside help. Some of the most frequently used for our office are listed below, but always consult Rae Frisch in our business office and obtain your forms the RIAS website to be sure you using the most up-to-date forms.

Submit all completed forms to the Business Office.

Hiring a writer, photographer, or illustrator

  • Independent Contractor or Employee Determination Form (ICED)
  • Completed W9 (sometimes we already have this on file; check with the business office)

Hiring a graphic designer

  • Professional Service Provider Agreement
  • ICED
  • Completed W9 (sometimes we already have this on file; check with the business office)

Hiring a type 4 employee

  • Discuss this with the Business Office and find out whether or not he/she is already in payroll. If not, you will need the individual to provide:
    • Copy of SS card Copy of photo id 
    • Completed W4 Completed I9
    • Completed Rutgers Direct Deposit Form

All these go to the business office prior to hiring the person. They will let you know when the paperwork has been processed. If already on payroll, just have the employee submit invoices weekly or biweekly. Full-time type 4 employees should not work in excess of 40 hours per week, and part-time type 4s may not work more than 20 hours per week. Type 4 employees must make the same hourly rate from project to project, even if the work they have done previously is for a different department.
